Atypical is dedicated to maintaining the privacy of your protected health information (PHI). In conducting our business, we will create records regarding you and the treatment and services we provide to you. We are required by law to maintain the confidentiality of health information that identifies you. Atypical will always post a copy of our current Notice in our offices in a visible location, and you may request a copy of our most current Notice at any time.
You have the right to:

• Correct your paper or electronic medical record
• Request confidential communication
• Ask us to limit the information we share
• Receive a list of those with whom we have shared your information
• Receive a copy of this privacy notice
• Choose someone to act for you
• File a complaint if you believe your privacy rights have been violated

• Bill for services we provided
• Do research
• Comply with the law
• Address workers’ compensation, law enforcement, and other government
requests
